To complete the address change on your child(ren)'s school record, you must attach a copy of your new lease, mortgage, or tax statement containing your name and new address as well as a copy of one of the following: a utility bill, satellite TV/cable bill, or landline phone bill containing your name and new address. If you are living in a relative's home, please read the new student residency document for further instructions. (Please know that you might also be required to show your original documents to the school secretary once school is back in session in the buildings.)

Residency Affidavit: I declare that I sleep and eat at this residence and will be available for contact by officials of the South Redford School District at this address. I also declare that I am in compliance with the General School Laws of the State of Michigan, including Section 380.1148, which forbids placing a child "for an educational purpose" in a school district in which the parents or legal guardians of that child are not legally residing if open enrollment for my child is not an option. I further understand that if statements made on this form change, or are proved to be untrue, my child/children's enrollment will be immediately terminated in the South Redford School District; and I will be liable for ALL COSTS incurred while my child was illegally enrolled in the South Redford School District.
